To Schedule a Virtual PHA:

 

1.Complete PHA Part 1 online before scheduling

–Visit: Medical Operational Data System (CAC required, must be on gov. network)
--Select CAC enabled
--Select MHA
–Select “Self Service” then select the link for “Periodic Health Assessment”

2.Call the Virtual Medical Center at 1-844-863-3236

–Press 1 to schedule your virtual PHA
–Be prepared to provide your UIC, DOD ID, DOB, and completion date of PHA Part 1
