Consolidated Troop Medical Clinic (CTMC)
We are the Primary Care Manager for the majority of Soldiers stationed on Main Post. The CTMC offers a larger range of services to include; Primary Care, Preventive care, Aviation Medicine, Dive Medicine, Sports Medicine, Behavioral Health, Optometry , Occupational Therapy, Physical Therapy, Medical Management Center and Physical Exams for schools. We also have many other services to help support the Soldiers of Fort Moore that include; Laboratory Services, Pharmacy, Radiology and daily updates for MEDPROS.
We are dedicated to offering the best health care for our Soldiers by continuing to improve. We are embracing the guidance given by The Office of the Surgeon General and creating a Patient Centered Medical Home that will offer a Soldiers Centered Care for our War Fighters.
Harmony Church Troop Medical and Dental Clinic (HCTMDC)
Provides acute and longitudinal outpatient care to student and cadre active duty soldiers assigned or attached to the Armor School. Care includes all illnesses and outpatient procedures normally treated by credentialed board eligible or board certified family physicians and supervised physician assistants. Acute, routine and follow-up care is provided via same day walk-in appointments as well as other services that include behavioral health care, physicals therapy and routine physicals. The HC-TMDC will perform other medical support missions as directed by the Hospital Commander.
30th AG
30th AG Reception Battalion processes new recruits into the Army from Active Duty, Reserves, and National Guard. We mini-register new recruits into Health Care Systems (CHCS) and compile their initial medical record. Orders are then placed for laboratory specimens to include various titers, DNA, Blood Type and HIV. The recruits then receive the required baseline vaccinations and blood draws on Day 1 of processing. On Day 3 they return for PPD reading as well as any needed vaccinations indicated by their titer reads. All of the received vaccinations are then entered into MEDPROS as well as in an AHLTA encounter. We also screen for outdated physical exams and update profiles that are incorrect from MEPS. We identify individuals who do not meet entry level standards under AR 40-501. This past year we processed approximately 37,000 soldiers.
We are the only Reception Battalion that sees sick-call for those soldiers processing. Acute care is provided by an open access walk-in system. We devise treatment plans based on history, physical exam and laboratory or radiological studies to determine duty status; referring to a higher level as indicated. Also the staff is tasked to provide support to DWC details and weekend sick call.
TMC 4
Provides acute and longitudinal outpatient care to active duty Soldiers assigned or attached to 7th SFG (A) and to eligible individuals training or working with 7th SFG (A) at Eglin AFB, FL. Care includes all illnesses and outpatient procedures normally treated by credentialed board eligible or board certified Family physicians, emergency medicine physicians, internists and supervised physician assistants. Acute, routine and follow up care is provided via same day walk-in appointments and physicals are scheduled by appointment. Other services offered include behavioral health care, physical therapy, acupuncture, USA John F Kennedy Special Warfare Center & School physicals, Aviation physicals and other routine physicals. The clinic also has an extensive Soldier Readiness Process mission with a focus on Soldier readiness and a continuous deployment cycle for 7th SFG (A). Emergencies are referred to emergency services at local hospitals. When a patient's care exceeds the capabilities offered by TMC #4, the patient is referred to 96th Medical Group at Eglin AFB or to TRICARE contract services for outpatient specialty care.
TMC 5 (Inactive)
TMC 6 (Dahlonega, GA)
Provides acute care and outpatient services to the soldiers attending the US Army Ranger School and the cadre members who train them for the 5th Ranger Training Battalion. Ranger school is broken up into three phase. Each phase is approximately 21 days long. The initial phase is in Fort Moore, Georgia. The next phase is here in the mountains of North Georgia at Camp Frank Merrill and is considered the "Mountain Phase". The final phase of Ranger School is in Camp Rudder, Eglin AFB, Florida. TMC #6 supports all operations and provides healthcare to any sick and injured student or instructors (if needed) through on-call medics, 24 hour TMC manning and ambulance coverage at certain events. This coverage is spread across two training sites; the primary site in Dahlonega and a secondary site for mountain climbing specific training in Cleveland Georgia at Yonah Mountain, approximately 40 miles east of Camp Frank Merrill. Routine care and physicals are provided by appointment while acute or emergent care is provided on an open access walk-in basis (TMC #6 does not provide physicals for the cadre on camp the cadre go to their off post providers, the cadre must see their provider for routine care). Emergencies that exceed TMC #6’s capabilities are referred to emergency services at local hospitals (Chestatee Regional Hospital and Northeast Georgia Medical Center) on a “case by case” basis
TMC 8 (Eglin AFB, FL)
Provides acute care and outpatient services to the soldiers attending the US Army Ranger School and the cadre members who train them for the 6th Ranger Training Battalion. Ranger school is broken up into three phase. Each phase is approximately 21 days long. The initial phase is in Fort Moore, Georgia. The next phase is the mountain phase and taught in Dahlonega, Georgia and is supported by TMC #11. The final phase of Ranger School is in Camp Rudder, Eglin AFB, Florida. It is called “the Swamp Phase”. TMC #8 supports all operations and provides healthcare to any sick and injured student or instructors (if needed). Routine care and physicals are provided by appointment with their Primary Care Provider at 96th Medical Group, while acute or emergent care is provided on an open access walk-in basis at TMC #12. Emergencies the exceed TMC #8s capabilities are referred to emergency services at local hospitals (96th Medical Group at Eglin AFB or Fort Walton Beach Medical Center) on a “case by case” basis.
Sledgehammer TMC
Provides comprehensive quality primary outpatient care to eligible active duty Soldiers assigned to the Security Forces Assistance Brigades, 3-16th Armor and 194th Infantry Brigades of Fort Moore. Superior healthcare is provided to the patients within a Soldier Centered Medical Home Model, and is organized to meet soldiers' individual healthcare needs and readiness. Outpatient medical care is provided for acute and chronic illnesses by trained and credentialed Family Practice Physicians, and Physician Assistants. Acute care is provided by an open access walk-in system, and routine care by appointments (including physical exams).
Winder TMC
Located on Sand Hill at Fort Moore, provides acute, wellness, and continuity care to IET (Initial Entry Training) Soldiers of the 192nd and 198th Infantry Brigades. Included in these brigades are the 14-week OSUT (One-Station-Unit-Training) Infantry Soldiers and other 9-week Basic Trainees with Support MOSs (Military Occupational Specialties) who will complete AIT (Advanced Initial Training) elsewhere. Over 35,000 soldiers are processed through Sand Hill yearly, and close to 100% of these soldiers are seen at Winder TMC for Sick Call, vaccinations, and Airborne / Special Forces Physicals. Also conducted are various administrative visits, separations, and Medical Boards. Due to the very high number of Soldiers that must be seen at Winder on a weekly basis, the “Flow Cell” protocol is utilized. Flow Cell, developed based on concepts of “Lean Six Sigma,” places the right job in the hands of the right person, eliminating wasted work and maximizing efficiency of all. This allows the vast majority of Soldiers to return to their units rapidly, preventing valuable missed training time. All Soldiers are seen on a walk-in basis. Winder TMC also includes several auxiliary departments including Optometry, Physical Therapy, Occupational Therapy, Laboratory, and Radiology thus often permitting one-stop care and same-day disposition.
Medical Aid Facility (Fort McClellan)
The Medical Aid Facility (MAF), located at Fort McClellan ARNGTC (Army National Guard Training Center), is the consolidated center for all routine and non-life threatening emergency medical care at the training center to include Annual Physical Health Assessments. It is also the place to call for help in resolving any medical issues or questions that may arise while you are here at the Training Center. During the published AT season, the MAF may be staffed by either Active Component (AC), Reserve Component (RC), or civilian medical personnel selected specifically for AT site support. Martin Army Community Hospital, Ft Moore, Ga, credentials all the health care providers i.e., Physicians, Physicians Assistants, and Nurse Practitioners who provide medical care for soldiers at the MAF.
The MAF is well equipped and staffed to handle all of the routine illnesses and injuries that occur during AT. We can handle most emergencies, except life threatening emergencies. The MAF does not arrange for and/or transport the soldiers for routine civilian medical care and emergency dental care. Civilian care would include lab tests or X-rays at a local hospital or specialty visits arranged by the MAF. Emergencies will be transported to a local ER by Civilian Ambulance Service.
